When it comes to healthcare, you can expect more from Community Memorial. That's why we've launched MyChart – our new, easy to use patient portal.

MyChart lets you and your healthcare providers see all your health information in one place – across our Hospitals, Health Centers, pharmacy, lab, and long-term care facility. Your information is secure, seamless, and easy to manage on one simple application.

With MyChart you can:

Send messages to your doctor and their office staff
Schedule and check-in for appointments and complete pre-visit tasks
View your medications
View lab results
View and pay medical bills ad get price estimates
Take care of your children and other family members – all from one account!

Get Started with MyChart on Your Mobile Device

On your mobile device, open the Apple App Store (on iOS) or the Google Play Store (on Android), and search for MyChart. Look for the red MyChart logo to make sure you have the correct app.
Tap Install.
After you've installed the app, tap Open or find the MyChart icon (see right) on your device and tap to open it.
Select Community Memorial Healthcare from the list of organizations. If you don't see it right away, you can search by name, state, or zip code.

Get Started with MyChart on Desktop or Laptop

1. Click Access MyChart.

2. Once there, click Access MyChart under Sign Up/Sign In to MyChart.

3. If you already have a MyChart account, enter your username and password to sign in.

4. If you are setting up a new account, click Sign Up Now, then enter your email address. You will be given a verification code via email, and then asked to set up a password. After creating your password, log in and select Community Memorial Healthcare as your organization.

Recover a Lost Username/Password or Get Support

If you're having trouble logging in, click the Forgot MyChart Username? or Forgot Password? link below the login fields for assistance. If you forgot your username, you will need to enter your identifying information. If you forgot your password, you will need to enter your username, and a verification code will be sent to your email to reset your password.

FollowMyHealth^® Patient Portal

FollowMyHealth is a free health management tool that helps you manage your health care from your computer, tablet, or smartphone. You can schedule appointments, view test results, pay your bills, and much more! In addition, you will be able to see your health information from Community Memorial Hospital in Ventura and Ojai.
