Your doctor of nurse will tell you when you will be discharged and give you instructions for follow-up visits and home care. The official discharge time is 11:00 am. Be sure to collect all your belongings. If you have had valuables stored in the Admitting safe, they will be released to you at discharge.
If you require continuing care, our Social Services team will help with planning for your care when you are discharged from Community Memorial Hospital. They will discuss your options regarding local home healthcare agencies, convalescent hospitals, medical equipment rentals, portable meals, and transportation. Our licensed social workers are also available to help you with personal or family problems, or the emotional demands of sudden or chronic illness. There is no charge for this service.
Focused on comfort and convenience in your transition to home, our Meds-to-Beds program is designed to bypass the need to stop at a pharmacy upon discharge. This courtesy service is offered by Community Memorial Pharmacy in Ventura. Our pharmacy will process and fill your prescriptions as well as discuss your medications with you prior to your discharge. What's more, your prescriptions will be delivered directly to your bedside prior to leaving the hospital.
When you are ready to leave, a hospital representative may escort you in a wheelchair to the car.
