Supportive Care services at Community Memorial go beyond traditional inpatient and outpatient medical care to help you play an active role in your own care or that of a loved one. Whether you need help managing a chronic condition, formalizing your end-of-life care preferences, or navigating the challenges of being a caregiver, we’re here to support you.

Supportive Care Services

  • Advance Care Planning: Help documenting your wishes for care and life support should you become incapacitated and unable to make such decisions on your own.
  • Caregiver Navigator Program: Resources and support for people who care for patients with complex medical illnesses, mental health issues, substance use disorder, Alzheimer’s/dementia, or developmental challenges.
  • Caregiver Support Group: Caring for a loved one with medical, psychological, substance use, or developmental needs can feel overwhelming, exhausting, and isolating. Our Caregiver Support Group offers a safe space for caregivers to pause, focus on self-care, and connect with others who understand similar challenges. The group meets Thursdays from 2:50 pm–4:00 pm at Community Memorial Hospital – Ventura in the Ocean Tower lobby. Please call 805-948-5803 for more information.
  • Chronic Care Management: Help for people managing one or more chronic medical conditions and dealing with a complex care plan.
  • Dementia Care Management: Education, support, and programs for individuals with dementia, including screening and placement in specific programs.
  • Diabetes Self-Care Management Education: Education and support for anyone with a Type I or Type II diabetes diagnosis who wants to learn how to best manage their condition and improve their overall quality of life.
  • Enhanced Care Management: Extra care coordination for Gold Coast/Medi-Cal members with complex health and social needs. This program can help patients better understand their care, connect with services, and feel more supported as they manage their health.
  • Palliative Care: Medical care focused on pain and symptom management and enhancing quality of life for those facing serious illness.
  • Spiritual Care: Non-sectarian emotional and spiritual support for patients, their families, and friends.
  • Our Substance Use Navigator program provides compassionate, judgment-free support for people who are struggling with alcohol, opioids, or other substances, helping them understand their condition, start treatment, and connect with follow-up care.
