COMING SOON! MyChart – Your Easy, Secure Patient Portal
In May 2025, we will be rolling out MyChart, an easy, secure way to manage
your healthcare needs. This powerful technology can be used on a computer
or smart phone to create a seamless experience throughout your entire
health journey, whether you are being seen in a clinic or at the hospital.
Your providers can immediately access all of your health information in
one place, so they can make more informed decisions about your diagnosis
and treatment plan. And you’ll have lots of great benefits from
this single medical record portal.
You can complete pre-visit tasks, view test results, medication lists, upcoming appointments, medical bills, price estimates, and more all in one place using the app on your phone or computer.
Connect With Your Doctor No Matter Where You Are
Through MyChart, you can send a message or arrange a follow-up appointment with your doctor, depending on the level of care you need.
Take Care of Your Children and Other Family Members
Stay on top of test results, appointments, and check in on family members who need extra help, all from your account. Parents and guardians can request proxy access completely online through MyChart. Proxy access allows parents and guardians to view health information, update insurance and utilize self-service tools such as eCheck-In.
Schedule Appointments
Online scheduling is available for our Community Memorial Health Centers via the MyChart app. Scheduling is easy to use. A calendar pops up with the available dates and times, you choose the one you want, and your appointment is instantly confirmed–no need to wait for the clinic to get back to you. You can also easily cancel or reschedule existing appointments.
Billing & Payments
Access your billing statements, review your payment history, and pay your bill online via MyChart. Once logged into your MyChart account, simply click on the navigation menu and scroll to the Billing section to get started. Credit card payments can be made directly and securely through MyChart to your Community Memorial Health Center or Physician Office accounts. You can also make a payment as a guest without logging in, here.
